Instructions on completing your application are below.

Passport Checklist

Fill Out an Application in Printed/Black Ink ONLY.

Do Not Sign the Application Until Instructed To Do So By The Acceptance Agent.

• You must provide a Social Security Number (SSN) in accordance with 22 U.S.C. 2714a.
• You may request a larger passport book with 52 pages, at no additional cost, by checking the '52 page' box at the top of the DS-11.

Provide Evidence of U.S. Citizenship

You must submit one of the following. Your evidence must be an original or certified, physical copy. Photocopies or Abstracts of Birth are not acceptable. 

• U.S. birth certificate that meets the following requirements:
  • Issued by the city, county, or state of birth
  • List your full name, date of birth and place of birth
  • List your parent(s)' full names
  • Has the date filed with registrar’s office (must be within one year of birth)
  • Has the registrar’s signature
  • Has the seal of the issuing authority
• Fully-valid, undamaged U.S. passport (may be expired)
• Consular Report of Birth Abroad or Certification of Birth
• Certificate of Naturalization
• Certificate of Citizenship

Your evidence will be returned to you if it is not damaged, altered, or forged.

Present Identification

You must present one of the following identification documents (IDs) to the acceptance agent. The ID must readily identify you. 

• In-state, fully-valid driver's license
• Full-valid, undamaged U.S. passport (may be expired) 
• Certificate of Naturalization or Citizenship
• Government employee ID (city, county, state or federal)
• U.S. military ID or military dependent ID
• Valid foreign passport
• Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)

You must submit your: completed application, including citizenship evidence, ID, photo, and fees, in-person to the Santee City Clerk's Office.

Passport Photos

We are excited to now offer Passport Photos at the City of Santee. Passport Photos cost $20.00 per person and below are some photo basics to help make sure you are prepared for your photo.

1. Have a neutral facial expression or a natural smile, with both eyes open.
2. You cannot wear glasses
3. Taken in clothing normally worn on a daily basis. (no uniforms or camouflage attire)
4. You can wear jewelry and keep on your facial piercings as long as they do not hide your face.

Important Information

Fees to the Department of State are payable by check or money order only.

The City of Santee processes passport applications by appointment ONLY. If you need to renew a passport, please review the guidelines on the U.S. Department of State’s website here to ensure you can apply in person and do not have to renew by mail. Please review the requirements to obtaining a passport through a passport acceptance agency.

You will need two checks or money orders, one made payable to the U.S. Department of State and one made payable to the City of Santee. The U.S. Department of State DOES NOT take cash OR credit card payments. Money Orders can be purchased at various locations in the City.

Passport hours are Monday through Thursday from 8:30 am to 4:00 pm and Friday from 8:30 am to 12:00 pm
